Severe Mental Illness (SMI)

To address the mortality gap between patients on the SMI register and the rest of the population, NCL ICB have invested in the SMI Health Check project to address physical health concerns. This work is being completed in collaboration with our colleagues at the Camden and Islington Mental Health Trust. The clinical team (2 nurses, 2 HCAs) offer a physical health check, care planning and onward referrals where appropriate. Our KPIs are focused on the commitment to reach target of 60% of SMI patients have ALL 6 PH checks:

1. Measurement of weight (BMI or BMI + weight circumference)

2. Blood pressure and pulse check (diastolic and systolic blood pressure recording + pulse rate)

3. Blood lipids including cholesterol test (cholesterol measurement or QRISK measurement)

4. Blood glucose (blood glucose or HbA1c measurement)

5. Assessment of alcohol consumption

6. Assessment of smoking status

SMI Cafes

The team that makes up the SMI service, was asked to take on the task of SMI Health Cafes in the borough. The cafés will encourage people with SMI to receive annual health checks and focus on whole population approaches to improve outcomes.  In addition to offering support for people to access / receive health checks at the cafes they will also offer a range of health and wellbeing initiatives / taster sessions to support residents such as information on healthy eating, exercise, smoking cessation, social networking, day opportunities and links to employment opportunities. The cafes will also provide an opportunity to listen to our local communities about how we can better tailor support and access services to reduce the barriers to uptake in particular communities / parts of the borough.  

These will be run once a month and will be hosted at CANDI’s new building at Lowther Road.
