What information do we hold?
We at Islington GP Federation aim to provide you with the highest quality of healthcare. To do this we must keep records about you, your health and the care we have provided or plan to provide to you.
The doctors and other health professionals and our staff who care for you keep records about your health and treatment so that they are able to provide you with the best care and these records also aid the continuity of care you receive from the team.
This record is called your ‘health care record’. It may be stored in paper form or on computer and electronic systems and may include basic personal details about you, such as address, date of birth, NHS number and next of kin as well as sensitive personal data such as the contacts we have had with you, clinical visits, notes and reports about your health and treatment and investigation results such as x-rays, laboratory tests etc. It may also contain information sent to us about you from other organisations involved in your care.
Healthcare providers are permitted to collect, store, use and share this information under Data Protection Legislation which has a specific section related to healthcare information.

In accordance with Data Protection legislation & the Access to Health Records Act patients may request to see the information held about them. This is called a Subject Access Request. View our Subject Access Request Policy.
