Advise for patients after insertion of an intrauterine Device (IUD) or Intrauterine System (IUS)

Intrauterine devices are extremely effective methods of contraception. Please follow the advice below after insertion of a device:
The pain people feel following insertion varies greatly from nothing to strong period-like cramps. Pain may continue for a day or two following insertion. Simply analgesia such as Paracetamol or Ibuprofen can be taken to control the pain, if you are not allergic to these medications.
If you are experience a severe attack of acute pain, or any continuous, lower abdominal pain you should seek medical advice.
It is recommended that you wait about 3 days after fitting before having sex, to reduce the risk of infection.
Use of sanitary pads is recommended until your next period. Thereafter if tampons are used, you should change them with care. Mooncups can also be used however there have been reports of coils coming out due to the suction forces so please take care.
You may experience some prolonged periods following insertion and initially you may experience some bleeding in between your periods, which should settle within three to six months after the procedure.
If you are experiencing any symptoms of infection, you need to be seen as soon as possible. Symptoms may include any of the following:
Persistent or worsening lower abdominal pain
Abnormal vaginal discharge
A shivering attack or otherwise feeling unwell with fever
Pain during sexual intercourse
If you have any of the above symptoms, either attend your local Contraception and Sexual Health Clinic or visit your GP. If those options are unavailable, attend your Local A&E Department.
You should feel for the threads of the IUD/IUS at least once a month. Gently put your finger into your vagina and feel for the strings at the cervix.
If you cannot feel the threads or you can feel the stem of the coil, you must assume that you are no longer protected, start using another method (e.g. condoms) and make an urgent appointment to be seen and checked.
Expected changes to your bleeding pattern with the IUS (Mirena)
In the first 3-6 months after insertion of an intrauterine system (IUS) irregular and unpredictable bleeding is common. This is rarely heavy and should settle with time. After this time, the majority of women will have light, infrequent periods or no periods.
